This exhibit consist of two procedures: Pain Management and Permanent Spinal Cord Stimulator Implant. To manage pain, bilateral facet joint injections are applied to L3-4, L4-5 and L5-S1, sacroiliac joint injections are also applied multiple times. In the second part, a spinal stimulator is implanted under the skin in the left buttock and an 8-contacts peripheral lead wires is placed in the paraspinal area of L3 through L5 and a 4-contacts wide spread, Quadripolar leads are introduced through the T12-L1 inter laminar space and guided in the epidural space up to the T7-8 level, then all this four wires are passed under the skin and connected to the spinal cord stimulator.
