The third illustration shows the first step in the procedure, in which the pylorus is opened to allow access to the duodenal ulcer. The fourth illustration shows the clamping and suturing of the actively bleeding vessel in the ulcer. The fifth illustration shows the transverse closing of the pyloric incision to widen the opening of the duodenum. the sixth illustration shows the excision of segments of the vagus nerve to inhibit acid secretion in the stomach.
